健走運動對社區中老年人心血管危險因子之影響

Effect of Brisk Walking Program on Cardiovascular Risk Factors in Middle-Aged and Older Adults in the Community

摘要


本研究目的是以傳統血脂、佛來明罕危險分數(Framingham risk score, FRS)與高敏感C-反應蛋白(highsensitivity C-reactive protein, hs-CRP)為指標,評估以社區健康營造的方式進行每週三到五日、每日30分鐘健走運動對國內社區中老年人心血管健康的效益。33名平均年齡62.3±10.27歲、至少一年以上無規律運動習慣之社區健康老年人為受試者,採立意取樣方式編組,分派為健走組及控制組二組,健走組以每週二次社區健康營造工作坊安排健走運動指導課程,另有三天社區自行結伴練習,以達到每週150分鐘的運動時間,控制組則是與運動組在年齡、身高、體重及身體質量指數相近,且無規律運動習慣者。蒐集二組受試者在20週健走介入前後包括血壓、體重、腰圍、身體質量指數(BMI)、FRS指數、總膽固醇(total cholesterol, TC)、三酸甘油脂(triglycerides, TG)、高密度脂蛋白膽固醇(high-density lipoprotein cholesterol, HDL-C)、低密度脂蛋白膽固醇(low-density lipoprotein cholesterol, LDL-C)、空腹血糖(plasma glucose)、hs-CRP的改變。以二因子變異數分析(two-way ANOVA with repeated measures, exercise × time)考驗各依變項在健走運動介入前後時間組間的變化。本研究之結果:經過20週的健走運動後,與控制組相比較,健走組在BMI、腰圍、血壓、TC、TG皆有顯著下降(p<.05)與維持HDL-C、FRS與十年發生率數值的穩定(p<.05),在LDL-C、PG與hs-CRP的改變上,則二組間無顯著差異。本研究之結論:以健康營造為策略、進行20週、每週至少三次,30分鐘3,780步以上的社區健走運動,雖因運動強度不足,對降低心血管發炎指標hs-CRP效果不明顯,但可以改善老年人的身體組成、血壓、血脂質等心血管危險因子,降低冠心病的發生風險,是值得作為社區推展預防心血管疾病之健康營造促進活動。

The purpose of this study was to examine the influence of a 20-wk community based brisk walking program on blood pressure (BP), body mass index (BMI), lipid profiles, Framingham risk score (FRS) and high-sensitivity c-reactive protein (hs-CRP) in middle-aged and older adults. Thirty three subjects aged 63.2±10.27 years were assigned into either walking group or no-walking (control) group. Walking group participated in five 30-minute brisk walking/week in a worksite based walking programs of community health planning. Fasting lipids including total cholesterol, high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglycerides (TG), glucose, hs-CRP, BP, FRS and anthropometric characteristics were measured at baseline and follow-up 20 weeks. The t-test and two-way ANOVA (exercise× time) were used for data analysis. In this study, compared to the control group, walking group showed a significant reduction in body weight, BMI, waistline, BP, TC, TG and HDL-C, FRS (p<.05). No significant changes were shown for other risk factors. These findings indicate that participation in 20 weeks, at least three times a week of 30 minutes of 3, 780 steps of brisk walking program can improves body composition, reduces BP, lipid profiles and prevent an increase in risk of cardiovascular disease in community sedentary meddle-aged and older adults. Due to low exercise intensity, no obvious decrease of cardiovascular inflammation index hs-CRP was noticed in our results. However, it is still worthy of promoting community exercise programs to prevent cardiovascular risk factors.
