經皮穿刺椎體成形術治療疼痛性骨質疏鬆之脊椎壓迫性骨折

Percutaneous Vertebroplasty for Painful Osteoporotic Vertebral Compression Fractures

摘要


骨質疏鬆造成脊椎壓迫性骨折是大於50歲族群疼痛及活動受限常見的原因,有很高機會造成續發性的併發症及死亡。在台灣每年約有將近五萬人的新增病例,平均每人一生中有高達四分之一機會可能會遭遇這樣的問題。雖然多數骨折會在幾個月內癒合,但是有部分病人在接受保守治療後,卻仍持續疼痛及活動不良,有些病人進而需要住院治療,甚至須要長期照護。對這類的病患而言,積極的治療,有效的控制疼痛方能避免長期臥床並縮短病程。經皮穿刺椎體成形術是近幾年來較被大家接受,較不具侵犯性的治療方法,主要是能穩定骨折壓迫的椎體,進而減少疼痛及增進患者功能的恢復。本篇文章的主旨,主要是藉由系列的文獻回顧,來探討經皮穿刺椎體成形術治療疼痛性骨質疏鬆之脊椎壓迫性骨折的臨床成效,以提供臨床醫師作爲治療病患的參考。

Osteoporotic vertebral compression fractures are a common cause of pain and disability and associated with increased morbidity and mortality. Approximately 50,000 new vertebral fractures occur in Taiwan each year, and it is estimated that up to a quarter of people who are older than 50 years of age will experience at least one vertebral fracture in their lifetime. Although most fractures heal within a few months, some people suffer persistent pain and disability and fail to respond to conservative therapies. Some may even require hospitalization, long-term care, or both. Therefore, interventions that effectively manage pain and shorten recovery time would be of great benefit. Percutaneous vertebroplasty, a minimal invasive technique developed more than a decade ago, has been getting popular in recent years in treating painful osteoporotic vertebral compression fractures. Early studies reported promising results on immediate pain relief and functional improvement by stabilization of the collapsed vertebral body. The purpose of this study is to assess the efficacy and safety of percutaneous vertebroplasty as a treatment for painful osteoporotic vertebral compression fractures by means of a systematic review of the current literature.
