摘要

Caldwell-Luc氏手術是一個相當具有歷史地位的手術，它曾經是治療慢性鼻竇炎的主流手術，現在已被鼻竇功能性內視鏡手術所取代，本文藉由文獻回顧探討它於現代醫療環境中的角色。傳統的Caldwell-Luc氏手術應包括唇下路徑進入上頜竇及永久性的下鼻道鼻竇造口術，有四個重要的功用：根治慢性上頜竇炎、移除上頜竇深處的病變、提供手術路徑及進一步成為Denker氏手術。雖然現在大部分的功能均已被內視鏡手術所取代，永久性的下鼻道鼻竇造口術已不再被鼓勵，但是，Caldwell-Luc氏手術之唇下路徑進入上頜竇及根治上頜竇病變的優點仍無法被取代。其實，結合兩種手術可以根治複雜嚴重的慢性鼻竇炎、解決彼此的死角得到更廣大的手術視野，甚至成為「微創性Caldwell-Luc氏手術」。

Caldwell-Luc's operation is a historic surgery. It was a major treatment for chronic sinusitis in the past. However, it has been replaced by functional endoscopic sinus surgery. We discuss its role in this modern medical era in this article by reviewing the literature. Traditionally, it includes sublabial antrostomy and permanent inferior antrostomy. It has four important indications: radical treatment of chronic maxillary sinusitis, removal of lesion in deep maxillary sinus, a route for special aim, and preparation for Denker's operation. The majority of indications were treated with endoscopic surgery, and permanent inferior antrostomy is not encouraged now. However well exposure and radical treatment of maxillary sinus are unique in Caldwell-Luc's operation. Therefore, cooperation of endoscopic surgery and Caldwell-Luc's operation can be indicated to treat severe diffuse chronic sinusitis, to get a broader view, and even to become a modified (”mini”) Caldwell-Luc's procedure.
