中醫益氣化瘀法對周邊神經病變的治療探討：一病歷討論

目的：肢體麻木是周邊神經病變其中一項特徵，而周邊神經病變能由許多不同的病因所造成，如：感染性疾病、新陳代謝疾病等等。臨床上我們看到許多這類病人的主訴為肢體麻木而中醫辯證屬於氣虛血瘀者，可以使用”益氣化瘀法”來給予治療。方法：我們搜尋了一些資料包括：臨床病例、動物實驗，另外還有一篇我們的病例，是採用益氣化瘀法來治療頸椎神經根病變。藉由這些資料，我們試著想要了解益氣化瘀法作用在周邊神經病變上的機制是如何。結果：我們搜尋了PubMed、Medline+Journals@ovid、China National Knowledge Infrastructure這些資料庫，找到了8篇中文論文有關益氣化瘀法的中文期刊以及1篇英文期刊有關中草藥對於頸椎神經根病變治療的系統性回顧。結論：研究顯示益氣化瘀法對周邊神經病變的治療效果可能是來自對受傷神經元細胞凋零的抑制，以及促進細胞外間質中的花生凝集素結合分子對許旺細胞的引導這兩條路徑。然而，我們仍需要更多高品質的實驗證據來證實益氣化瘀法對周邊神經病變的治療效果。

Objective: Peripheral neuropathy, one typical symptom was numbness, might be caused by different kinds of disease from degenerative diseases to metabolic diseases. Many patients had a chief complaint of numbness of limbs with a Chinese medicine diagnosis of ”Qi deficient and blood stasis pattern” can use Yiqi Huayu method(which means tonifying Qi and removing blood stasis) to treat their diseases. Methods: Herein, we reviewed the researches of Yiqi Huayu method recent years which included case reports and animal studies. We also had one our case report that use Yiqi Huayu method on cervical neuropathy. We try to investigate the mechanisms of how does Yiqi Huayu method work on peripheral neuropathy. Results: We searched the PubMed, Medline+ Journals @ ovid, and China National Knowledge Infrastructure data bases and collected 8 Chinese studies about Yiqi Huayu method and 1 English systemic review about Chinese medicine for cervical degenerative disc disease. Conclusion: The effect of Yiqi Huayu method on peripheral neuropathy may caused by inhibiting apoptosis of the damaged neuron and promoting peanut-agglutinin-binding molecules in guiding the sprouting and extending of the Schwann's cells. However, we still need more high quality evidence to prove the effect of Yiqi Huayu method on peripheral neuropathy.
