摘要


腋窩全層皮膚缺損須有足夠組織覆蓋,以防止其下方重要結構暴露及日後疤痕攣縮,在此我們提出三個腋窩全層皮膚缺損之病例。當傷口穩定後均以肩胛旁肌膜皮瓣盡早覆蓋。此皮瓣手術剝離取得容易,解剖變異少;皮瓣長寬比可達三比一。它可供應薄且柔軟之組織以符合肩膀之活動需要。病人可早期活動。此皮瓣比一般補皮更可減低日後攣縮發生率。

A full-thickness defect of the axillary region demands adequate coverage to prevent exposure of the underlying structures and later contracture. Here we present 3 cases of a full-thickness defect of the axilla. We covered the wound as early as possible with a parascapular flap after the wound condition was stable. Good results were obtained. This flap is easy to harvest with little variation in anatomy. A flap with a length to width ratio of 3 to 1 can be raised. It provides thin and pliable skin to cover the axilla, and which also fits the dynamic requirement of the shoulder joint. The patient can mobilize early and splinting is not required. Also this flap decreases the contracture rate of the axilla as compared with a STSG procedure.
