Vertical Reduction Mammaplasty (Lassus Method) In Oriental Women

東方女性利用Lassus方式進行垂直縮乳正形手術之探討

摘要


背 景:有許多針對女性縮乳手術的方式已經發表,這些手術方式主要的核心目標在於減少乳房體積、矯正下垂的外觀,最終留下一個小且不明顯的疤痕。到目前為止,在台灣對於縮乳手術的報告相當稀少,亦未有利用Lassuss方式進行垂直縮乳手術的報告被提出。目的及目標:藉由病歷回溯,報告我們針對東方女性利用Lassus垂直縮乳手術方式進行縮乳之手術經驗及結果。材料及方法:自2004年8月至2014年12月,總共收集16例利用Lassus方式進行垂直縮乳手術之案例,透過臨床病歷回顧、手術報告及臨床相片分析報告義大醫院利用Lassus垂直縮乳手術方式進行縮乳之手術結果與併發症。結 果:16位接受垂直縮乳手術之東方女性,接受手術時平均年齡為30.3歲,平均每個乳房切除508克,平均失血量為91.7毫升,追蹤時間平均為十二個月。沒有任何的術後血腫或乳頭壞死發生,沒有乳房感覺喪失。兩例乳頭乳暈位置過高,一例增生性疤痕。術前的身體症狀(肩頸疼痛等),大於80%在術後可得到完全緩解。結 論:Lassus垂直縮乳手術為一種可靠且安全的方式。對於容易形成疤痕之東方女性,Lassus垂直縮乳手術擁有短而不明顯疤痕(相對於倒T型刀口而言,比較美觀),較低失血量,術後無重大併發症,維持長時間良好外型等優點。我們認為這是一個可作用東方女性縮乳手術選擇的良好方式之一。

Background: Many reduction techniques have been described for the female hypertrophic beast. Their common target is to reduce the breast volume and correct ptosis, whilst leaving behind inconspicuous scars. In those populations with a propensity towards the formation of hypertrophic scars, as in the Orientals, the scar-sparing vertical reduction technique represents an advantageous strategy. Aim and Objectives: The aim of the current study is to present our results, complication and modifications in 16 oriental female hypertrophic breast patients with the Lassus vertical reduction mammaplasty technique. Patients and Methods: This is a retrospective study of sixteen oriental female patients, with a mean age of 30.3 years, who underwent bilateral superior pedicle vertical reduction mammaplasty, based on Lassus method, in E-Da Hospital, between August 2004 and December 2014. Assessments were based on pre- and postoperative medical record, photodocumentation, clinical examination in outpatient settings and telephone/direct questioning. Results: The mean resection weight per breast was 508 grams, ranging from 100 to 925 grams, while the mean intraoperative blood loss was estimated at 91.7ml. The mean follow up period was 12 months. Of all the preoperative symptoms(shoulder pain, back pain…), more than 80% were completely relieved. No complications were encountered as regards the healing of the nipple-areola-complex. Revisional surgery was required due to peri-areolar hypertrophic scarring in one patient and high riding nipple in 2 further patients. There were no wound infection, dehiscence, hematoma, seroma, skin or nipple areola complex necrosis and sensory loss. The upper fullness settled down achieved after 2 ~3 months or more. The final outcome was excellent in all cases. Conclusions: The Lassus vertical reduction mammaplasty is a simple in design and performance, reliable and safe breast reduction technique, associated with minimal blood loss and very low complication rates. Especially for the oriental population, with the hypertrophic scarring predisposition, the vertical pattern constitutes a further distinct advantage compared to the wise pattern reduction mammaplasty.
