以「資訊系統接受後持續使用模式」探討住院病患使用床邊資訊系統之意願

Exploring the intension of using bedside information system for inpatients by Post-Acceptance Model of Information System Continuance

摘要


前言:「醫病合作關係」近來逐漸成為醫療服務的中心思想,隨著資通訊科技的進步,床邊資訊系統可協助了解病人需求與喜好,並支持及教育病人參與決策,成為達成提升醫療品質目標之重要工具。方法:為探討影響床邊資訊系統持續使用意願的重要因素,本研究參照「資訊系統接受後持續使用模式」(Post-Acceptance Model of Information System Continuance,PAM)與「計畫行為理論」(Theory of Planned Behavior,TPB)編訂具信度、效度問卷,以北部某區域醫院使用床邊資訊系統之骨科住院病患為研究對象,獲得有效問卷109份(回收率81.3%),並以SPSS22及Smart PLS2.0統計軟體進行資料分析。結果:住院病患對床邊資訊系統所感受到的一致性會顯著影響其對系統的感知有用性與滿意度(ß=0.81,t=21.83;ß=0.52,t=17.71);感知有用性會顯著影響滿意度(ß=0.36,t=2.92);滿意度與知覺行為控制會顯著影響持續使用意願(ß=0.51,t=6.32;ß=0.34,t=4.66)。結論:住院病患使用床邊資訊系統之「感知有用性」、「滿意度」與「知覺行為控制」會顯著正向影響其對床邊資訊系統的「持續使用意願」。建議醫院可進一步透過系統功能項目滿意度調查,系統介面與内容的定期更新維修,以及加強相關推廣與使用指引訓練,以提升住院病患持續使用床邊資訊系統的意願。

Introduction: Due to the advance of Information and Communication technology, Bedside information system (BIS) is becoming one of the important tools for enhancing the quality of care, while "doctor-patient relationship-centered" gradually becomes the mainstream of healthcare. Method: In this study, we developed a questionnaire based by Post-Acceptance Model of Information System Continuance (PAM) and Theory of Planned Behavior (TPB) which passed the reliability and validity tests to explore important factors affecting the continuance usage of BIS. 109 valid samples of orthopedic ward inpatients in a north area hospital were collected, with 81.3% effective recovery rate. SPSS 22 and Smart PLS 2.0 software were used to analyze the data. Results: The results show that the inpatients' consistency can significantly affect their perceived usefulness and satisfaction to the system (ß=0.81, t=21.83; ß =0.52, t=17.71); perceived usefulness significantly affect satisfaction (ß=0.36, t=2.92); both satisfaction and perceived behavioral control significantly affect continuance usage (ß=0.51, t=6.32; ß=0.34, t=4.66). Conclusion: In summary, this study confirmed that inpatients' continuance usage of BIS is significantly affected by their "perceived usefulness", "satisfaction" and "perceived behavior control". We recommended that hospitals should devote to update and maintain BIS regularly, conduct satisfaction survey, and provide promotion and education training to enhance inpatients' using willingness of BIS.
