淋巴樣間質性肺炎：一病例報告

摘要

一位9歲男孩因慢性咳嗽及持績發燒一涸半月而入院檢查。理學檢查發現肝脾腫大及輕微杵狀指。胸部X光呈現兩側性間質浸潤。經予抗結核藥治療4個月無效。病人接受開胸肺穿剌檢查證實淋巴樣間質性肺炎。術從投予類固醇治療6週，不幸併發細菌性肺炎及成人型呼吸窘迫症候羣而死亡。

A 9-year-old boy was admitted to the Veterans General Hospital-Taichung via OPD with chief complaints of chronic cough and persistent fever for more than one month. Physical examination revealed hepatosplenomegaly and mild clubbing fingers. Chest roentgenograms showed diffuse bilateral interstitial infiltrates. Blood gas analysis showed mild hypoxemia and liver function tests were compatible with chronic hepatitis. He was given anti-tuberculosis regimen for 4 months in vain. Open lung biopsy was performed and revealed lymphoid interstitial pneumonia. After operation, be was put on steroids for 6 weeks and then tapered off. Unfortunately, he developed superimposed bacterial pneumonia and died of adult respiratory distress syndrome.
