Neonatal Varicella-Report of a Case
摘要
新生兒水痘在臺灣是一種罕見的疾病。本文報告一個15天大的男嬰，因4天前全身開始出現水疱而住院。理學檢查發現該病嬰反應遲鈍、活力差且全身滿佈水痘皮疹，部分已有出血及潰爛現象。胸部X光顯示兩側肺野有彌漫性浸潤。皮膚切片顯示表皮層有水疱形成及出現多形核巨大細胞。血清學檢查發現病嬰的varicella-zoster virus(VZV) IgG抗體效價爲1:2(-)，病嬰母親爲1:128(+)。病嬰在新生兒水痘的診斷之下，立即授予IV immunoglobulin (400mg/kg/day)及acyclovir (15mg/kg/day)，但治療無效，病嬰不幸於翌日死亡。

Neonatal Varicella is an uncommon disease in Taiwan. This paper presents a 15-day-old male infant who suffered from vesicular-bullous eruption for 4 days. Physical examination on admission revealed an acutely ill infant. A widespread vesicular eruption covered the skin, some hemorrhage and necrosis was evident on the trunk and lower abdomen. Chest X-ray showed diffuse mottled infiltration over the both lung field. Skin biopsy showed bullous lesion with infiltration of giant cell in the epidermis. The varicella-zoster virus IgG titer of the patient and his mother was 1:2 (-) & 1:128 (+) respectively. Under the impression of neonatal varicella, IV immunoglobulin (400mg/kg/day) and acyclovir (15mg/kg/day) were given, but in vain. The baby's condition deteriorated rapidly and expired on the next day. We hope that this article will stimulate physicians to alert the occurrence of neonatal varicella when the mother have varicella shortly before delivery.
