口腔顎面囊腫長久以未一直秋認為是臨床上不容忽視的問題。因為小的囊腫大多無症狀發生,可是一旦大到某種程度時,就會引起一些徵候產生,包括(1)病人之顏面變形(2)說話、咀嚼或吞嚥困難(3)正常齒列之干擾。 1979到1983年5年間,高雄醫學院口腔顎面外科曾診治過232位口腔顎面囊腫病人,就收集之資料,統計分析如下 (1) 139位是男性,93位是女性,男女比例為1.5:1。 (2)年齡白出生7個月至78歲。有161位(69.4 %)病人分佈於10歲至39歲之年齡層。 (3)顎骨性囊腫156個(65%)、口腔顎面軟組織囊腫74個 (30.8 %)、與上顎竇相關之囊腫10個(4.2%)。 (4)所有240個口腔顎面囊腫中,以根尖牙周囊腫80個(33.3 %)為最多,其次是唾液腺囊腫66個(27.5%),含牙囊腫有40個(16.7 %)佔第3位。 (5)急慢性威染之囊腫有48個,以根尖牙周囊腫最多,佔41.7%。 (6)只有一位病人具有文生氏歡候(Vincent's symptom),主訴上唇部有麻木及成覺障礎。 (7)依據囊腫之種類、位置、大小,其處理方式如下:①剜出術②造袋術③竇囊切除術④一般外科切除。 (8 )232位口腔顎面囊腫病人中,有6位於手術復病灶複發,其中2位之病灶復發2次。
A cyst is a pathological cavity having fluid, semifluid or gaseous contents which are not created by the accumulation of pus. The cyst wall is frequently, but not always, lined by epithelium. Cysts of the oral and maxillofacial region are well recognized as clinical problems. Although many small cysts are asymptomatic, they may slowly enlarge and result in a number of problems . These include (1) abnormal contour of the face (2) difficulty in speech, swallowing or mastication and (3) interference with normal dentition. The present investigation is a statistical report on the analysis of 232 patients with cysts of oral and maxillofacial region. These patients were treated during a period of 5 years at the Division of Oral and Maxillofacial Surgery, Department of Dentistry, Kao-Hsiung Medical College Hospital from January , 1979 to December 1983. The results were summarized as follows : (l) Among the 232 patients there were 139 male patients and 93 female patients. The male to female ratio was 1.5:1. (2) The age range was from 7 months to 78 years. There were 161 patients (69.4%) whose ages were between the second and fourth decades. (3) The distribution of 240 cysts was: 156 (65%) jaw bone, 74 (30.8 %) soft tissues of the mouth, face and neck and 10 (4.2%) associated with the maxillary antrum. (4) The most common of the 240 cysts were apical periodontal cysts (n = 80 or 33.3 %), followed by salivary gland cysts (n = 66 or 27.5 %) and dentigerous cysts (n = 40 or 16.7 %). (5) The majority of the chronically and acutely infected cysts (n = 48) were apical periodontal cysts (n = 20 or 42.0 %). (6) Only one out of our 232 patients had Vincent’s symptom, with the chief com- plaint of numbness and sensory disturbances of the lower lip. (7) According to the types, locations and sizes of cysts , four surgical methods were used for the treatments of our patients : enucleation, marsupialization, antrocystectomy and surgical excision. (8) In a total of 240 cases treated at our hospital , there were 6 recurrent cases after the first operation. Two out of these 6 cases were second recurrences.
