Unilaterally Congenital Dimple of the Upper Lip - A Case Study
單側上唇先天性淺窩－病例報告
摘要
唇部先天性陷窩為一先天性唇部畸型，一般以口角及下唇部為多，極少發於上唇部，發生於上唇的病例，多見於正中位或兩側，單側之病例尤為稀少。此陷窩又因深度及有無分泌物流出而有不同的名稱，如dimple（淺窩），sinus（竇），pit（凹陷），及fistula（瘻）。無性別差異，有家族傾向及遺傳性，並常伴隨其他部位的畸型，如唇顎裂，裂懸壅垂及耳前陷窩等，其發生原因不明，但一般認為與唇裂發生的理論相同。其病理切片檢查，在已報告的病例中均為扁平鱗狀上皮覆蓋，有些病例則有唾液腺、毛囊、皮脂腺、甚至軟骨的出現。其處理方注多採外科切除，並可獲得良好的結果，本院口腔顎面外科，最近經驗－發生於上唇單側的稀有病例，特提出報告。病人為一九歲女孩，為高齡父親與智能不足的母親所生，根據病人姑母表示，病人為自然生產，出生時左側上唇即有一凹陷，從無分泌物或其他症狀出現，家族中沒有其他畸型的病史。病人於常規檢查中被發現，而於1983年8月17日到本院求診，檢查時發現，陷窩有2×6 mm長3 mm深，根據Millard報告，我們稱之為dimple。此淺窩位於左側丘比特弓頂點下方的紅唇區，向下延伸到口唇黏膜，形成一個切迹，另外病人左側鼻部及白唇區與右側比較，有發育不良的情形，除此之外並無其他異常情形，診斷為單側上唇先天性陷窩，病人在克太拉(ketamine)麻醉及局部浸潤麻醉下接受切除手術及V-Y plasty（V-Y整形手術），術後病況展順利，預後良好。

Rare congenital depressions of the lips are classified as dimple, pit, sinus, and fistula. These congenital lip abnormalities are described as occurring bilaterally and more commonly, on the lip commissure and lower lip. Dimples or sinuses of the upper lip are comparatively rare, and are usually found in the midline. It is therefore of interest to present a case of a unilaterally congenital dimple on the vermilion of the left upper lip. The clinical findings and treatment are described, and relevant literature is reviewed in the following article.
