A new automated blood cell analyzer, sysmex E 4000, was used to provide an index of red blood cell volume distribution width (RDW). The mean value of RDW was studied in 441 normal controls and in 173 patients with a known diagnosis of iron deficiency anemia (IDA), heterozygous thalassemia or anemia of chronic disease (ACD). The mean value of RDW was 41.2±3.4 fl in the 441 normal controls;47.4±7.0 in 94 patients with IDA;37.8±3.7 in 42 patients with heterozygous thalassemia and 40.5±6.2 in 37 patients with ACD. The difference of the RDW between the IDA group and the other 2 groups was significant (p>0.01). It was concluded that patients with microcytic (low mean corpuscular volume )and heterogeneous anemia(high RDW)who are without a history of early onset of anemia and splenomegaly can be treated with iron therapeutically if conventional diagnostic tools are not available or are refused by the patient. But responses should be checked within a month to determine the possibility of a combined condition and whether to continue such treatment.
