格狀變換視覺誘發電位(pattern shift visual evoked potentials, PSVEP)可偵測尚無臨床表徵出現的糖尿病視神經徑路損傷。本研究收集59位糖尿病人,分為兩組:其中46位是NIDDM患者,13位是IDDM患者,這些患者的矯正視力都在0.8以上,眼底檢查無視網膜病變、眼角膜、水晶體、玻璃體正常,無青光眼、無白內障、對照組有年老及年輕兩組,其性別及年齡分別與NIDDM及IDDM組配對。此外還以血糖值,HbA1c,患病時間,正中神經的運動及感覺神經傳導速度(MNCV,SNCV)做為參考數據。研究結果顯示:IDDM患者除右側N75波潛時以外,其餘P100,N140之波潛時及N75-140波間潛時均顯著延長,而且P100婆振幅也比對照組顯著降低。兩眼P100波潛時差值(ILD)則無顯著差異。NIDDM患者PSVEP之各波潛時及波間潛時均顯著延長,但P100波振幅和ILD與對照組比較無顯著差異。在正中神經傳導方面,MNCV之IDDM組與其對照組比較有顯著延遲,SNCV亦然。在本研究中,N75波潛時延長和HbA1c有正線性相關,尤其在NIDDM組為然。N75波潛時延長和HbA1c有正線性相關,以全部DM患者組最為明顯。此外N75波潛時延長 也和正中神經的SNCV有負線性相關。以往PSVEP的重點都放在P100,本研究卻發現N75與糖尿的患病時間及HbA1c有正相關性,應該加以重視。
Functional exploration of the optic pa-thways with pattern shift visual evoked poten-tials (PSVEP) has been rapidly accepted as a non-invasive method of investigation of diabe-tics. For this article, we conducted the PSVEPs study on 46 cases of NIDDM and 13 cases of IDDM. The peak latency, interpeak latency and evoked amplitude of P100 were analyzed in each case. For further correlation, the motor and sensory nerve conduction velocities of the median nerve, blood sugar, serum HbA1c, and duration of DM were measured simultaneous-ly. Two nondiabetic control groups which matched the age and sex of the NIDDM and IDDM groups were used for comparison. In the IDDM group, the results showed prolongation of all peak and interpeak la-tencies (IPL) except the peak latency of N75 on the right side. The P100 amplitude was re-duced as compared with the age-matched young control group. The interocular P100 la-tency difference (ILD) was not statistically sig-nificant between the IDDM group and the age-matched control group. Theresult of the NIDDM group revealed prolongation of all peak latencies and IPLs. THE P100 amplitude and ILD showed no statistically significant dif-ference between NIDDM and the age-matched control group. The prolongation of N75 peak latency exhibited a linear correlation with duration, HbA1c and median nerve SNCV.
