內頸靜脈擴張症─病例報告
摘要
靜脈擴張症用來描述靜脈不正常擴張的一個名詞，可以發生在全身很多不同地方。會發生內頸靜脈擴張，可能是胸縱膈或頸部有某種機械性阻塞而導致胸内壓上升所造成。但不幸地，其真正原因並不清楚。內頸靜脈擴張症是一種非常稀少的疾患。在英文文獻裡從1928年至今，包括本篇的兩個病例，總共才36例。一般不須特殊治療，除了美觀上及心理上的因素除外。病例一是一住六歲的小女孩經診斷證實後，經外科手術治療，效果良好。病例二是一位六十六的歲的婦人，因無特殊需要，僅做追縱。要診斷此種異常，超音波應是一種最適當的檢查方式，因它最安全、清晰與花費最少。

Phlebectasia, defined as abnormal venous dilation, may occur in a number of different sites. Two cases with definite diagnosis of inter-nal jugular phlebectasia were reported. The first case was a 6-year-old girl with a bulging mass on right neck for 3 years. Angiography and CT scan showed definite diagnosis, and the bulging mass was resected from the internal jugular vein (IJV). The second case was a 66-year-old female patient, also complained of a bulging mass on her right neck. Sonogram and CT scan also showed the same diagnosis. Because it did not bother the patient, she was just under close obser-vation. From the three kinds of diagnostic modalities, we found so-nography is an effective technique because of its clarity, safety and low cost.
