應用對凝血酶敏感度高之合成產色基質H-D-Phe-Pip-Arg-pNA(S-2238)之醯胺水解反應,分析深部靜脈血栓症(DVT)及肺栓塞症(PE)之異常活化部份凝血質時間(APTT)及血漿中之肝素濃度,以求與測出之肝素量及APTT值,與使用肝素量之統計學關係。結果顯示測出肝素濃度與APTT關係密切,其相關係數,r=.73。當將所有數據置於10×10座標紙上時,發現PE患者之諸點均落在以DVT患者諸點製成之平均曲線之左方,顯示PE患者之肝素治療需要量比DVT患者大。
An amidolytic method for plasma heparin assay using a synthetic thrombin-susceptible chromogenic substrate, H-D-Phe-Pip-Arg-pNA (S-2238), has been applied to the plasmas of deep vein thrombosis (DVT) and pulmonary embolism (PE) patients with abnormal activated partial thromboplastin generation test (APTT) in order to find out the statistical correlations between the detected heparin concentrations and the APTT values, as well as the heparin doses. The results showed that there was an excellent agreement between detected heparin concentrations and the APTTs, with a correlation coefficient, r=.73. When the data were graphically plotted, all points of PE were located beyond the mean linear curve made by points of DVT patients, suggesting an increased requirements of heparin in patients with PE.
