空腸克隆氏症：病例報告
摘要
克隆氏症是一種腸道發炎疾病，而空腸克隆氏症是其中較罕見之類型，由於缺少具有診斷性之代表性症狀，且疾病又具多樣性，使得確定診斷有其困難性。這是一個23歲男性「空腸克隆氏症」患者，起初，在門診就醫時都被當作「腸道急躁症」治療，因併發症到急診就醫，手術前仍無法正確診斷而臆斷為「腸道惡性腫瘤」。病人緊急接受剖腹探查，並將具有病變之部分空腸切除，住院期問恢復情形平順，於術後第七天出院。診療醫師面對長期受到水瀉、體重流失困擾之年輕人，應考慮到「小腸克隆氏疾病」的可能性。對於屬慢性疾病之克隆氏症，目前並無治癒之方法，內科藥物治療或外科手術治療均僅能緩解症狀或併發症。

Crohn's disease is a kind of inflammatory bowel disease. Crohn's disease involving exclusively th jejunum (jejunoileitis) is relatively rare. Because of lack of specific symptoms/signs and of various presentations, definitive diagnosis is difficult to make. Here, a 23 year-old male with such a disorder is presented. An initial impression of irritable bowel syndrome was made at our clinic and the tentative preoperative diagnosis was GI tract malignancy. Exploratory laparotomy and segmental resection of the diseased jejunum were performed. The patient had an uneventful hospital course, and was discharged on the 7th postoperative day. The possibility of jejunal Crohn's disease should be kept in mind when the physician confronts a young patient suffering from watery diarrhea and body weight loss persisting for a period. Crohn's disease is a chronic condition for which there is currently no curative treatment. Either medication or surgical intervention can alleviate symptoms and complications.
