全股關節皮替代性形成術
摘要
自1971年5月至1984年4月的13年期間，榮民總醫院已為各種不同股關節病患者，行了652次代替性關節成形術。此種手術在最近3年內，有顯著地增加。基於使用的關節形式及行後是否已有足够長之時間以供評估，我們選擇了226位患者的271個股關節，作為追踪研究的對象。其立即效果確實令人鼓舞，因無菌性鬆脫必需再行手術者，有5.53％。加上本系列患者中，有三個股關節併發感染性鬆脫，使失敗率增加6.64％。手術之適應症及禁忌症，亦在本文檢討範圍內。有關手術本身之重要步驟及基臺持則曾作論述。其功能方面則分為疼痛、肌肉力量及運動範圍、行走、功能等四項，作分別評估。其中以疼痛一項，改善之幅度很大。我們發現患者之一般健康情形，疾病本身，及局部之病態組織之狀況，皆與南工全股關節代替成形手術之結果，有密切的關係。更重要者，開刀技術、生物機械方面之因素，以及人工關節之設計與形式，皆為影響此種外科療法功能性結果之重要項目。

In the period of 13 years from May 1971 to April 1984, the Veterans General Hospital have performed 652 hip replacement arthroplasties for various hip disorders. The cases of total hip replacement arthroplasty are significantly increased in the recent 3 years. We have selected 271 hips for study based on the type of prosthesis we used and the postoperative period was long enough for follow up evaluation, the immediate results were encouraging. The revision rate for aseptic loosening is 5.53%. There were 3 hips with septic loosening. These make failure rate 6.64%. The indications and contraindications are mentioned. The functional evaluation is itemized top pain, muscle power and motion, walking, and function. We find that patient’s general condition, the disease itself and the condition of local diseased tissues are closely related to the result of THR arthroplasty. Furthermore the operative technique, biomechanical factors and the prosthetic devices are also the great contributors to the functional results of such a surgical modality.
