自1982年1月到1984年11月2年11個月間,為14位病人共15例因股骨遠位端髁上及髁間骨折做治療,我們以90度髁上鋼板及壓迫性鋼釘做開放復位及內固定術。經過4到24個月的臨床及x光追蹤檢查,平均4.4個月骨骼癒合。沒有骨折癒合不良或內固定物斷裂的病例發生。有1例傷口感染,1例骨骼感染,但在追蹤檢查皆已完全癒合。所有病例中達到良好機能及x光癒合者73%,此項研究顯示以髁上鋼板及壓迫性鋼釘做內固定的好處,這種手術的技巧與髖部壓迫性鋼釘的使用方法相似,骨科醫師皆比熟悉。 我們結論認為以髁上鋼板及壓迫性鋼板釘治療股骨髁上及髁間骨折的優點是固定方法簡單,容易操作,可以壓迫骨折斷端而得到穩定復定,避免骨折碎片分離,可以做早期膝關節運動。
From January 1982 to November 1984, 14 supracondylar-intercondylar fracture of the distal femur in 15 patients sranging from 18 to 81 years old in the Changhua Christian Hospital were treated by open reduction and internal fixation with a 90-degree supracondylar plate and screw device. Clinical and radiographic follow-up spanned four to 24 months with the clinic and x-ray union taking 4.4 months in average. There was no nonunion and no implant failure. One deep wound infection and one osseous infection occurred but the two cases cured at follow up. Excellent or good function with radiographic results were achieved in 73% of all cases. This series demonstrates the advantage of stable internal fixation using a supracondylar plate and lag screw over other type of fixation . The technique is readily mastered and analogous to use of the hip compression screw. The results suggested that supracondylar plate and lag screw fixation for supracondylar-intercondylar fractures of the distal femur is advantageous because the technique is simple the lag screw compresses the condyle, preventing the condyle from separating to achieve stability. This rigid and stable internal fixation permits early knee joint exercise.
