End to Side Anastomosis of Microvessels

微小動脈末端對側端吻合術之探討

摘要


微小動脈的末端對側端吻合術已日漸流行,但對此種吻合術的技巧,尤其是吻合角度以及動脈切開樣式尚未定論,我們利用80隻老鼠的側端骼總動脈進行末端(左骼總動脈)對側端(右骼總動脈)的吻合術實驗,此血管之大小平均為1公釐,實驗動物依60°及90°之縫合角度,以及對側端動脈行直線動脈切開或取走橢圓動脈組織分為四組,雖然各組之間的吻合血管立即暢通率及術後7天暢通率,以及動脈瘤的形成率均無統計上之差別,但在縫合時間上取走橢圓形組織的動脈切開不論是60°或90°比動脈直線切開者有顯著的縮短。由於60°的縫合角度,必須把末端斜切,而且長角必須縫合至側端動脈之十二點方向,在技巧上之要求較大。基於以上之研究,我們認為90°的吻合角度加上取走橢圓組織的動脈切開在微小血管的末端對側端吻合術上是值得採用的方法。

Eighty rats underwent a study of end to side anastomoses with different arteriotomies (slit versus ellipse) and different angles of anastomoses (60° versus 90°). The left iliac artery was servered and reanastomosed to right iliac artery as a model of end to side anastomoses. There was no significant difference in the patency rate and the incidence of aneurysm formation. But the group of elliptical arteriotomy with suturing angle of 90° had the shortest suturing time. Because the elliptical arteriotomy with anastomotic angle of 90° gives the best visualization for the surgeon and this is the method of end to side anastomoses we like to recommend.
