兒童患副甲狀腺功能過高合併兩側股骨頸骨折-病例報告並回顧文獻

Primary Hyperparathroidism Presenting as Pathologic Fracture of the Femoral Neck in a Child - Report of Case and Brief Review of the Literature

摘要


兒童的原發性副甲狀腺功能過高大多是因為副甲狀腺腺瘤所引起。這種疾病在小孩很罕見。它的臨床症狀以腹痛、結石、骨骼疼痛最為常見。本文所要報告的病例剛到本院時有股骨頸骨拆、腹痛、噁心嘔吐、發燒,起初以為她有化膿性關節炎,入院檢查才知道她副甲狀腺功能過高,後來又發現她甲狀腺腫大,而且可以摸到腫塊。副甲狀腺瘤切除後,證實是副甲狀腺腺瘤。手術後的臨床症狀有顯著的改善。她的X光片以明顯的幹骺端緻密帶最為特別,與重金屬中毒的X光片有點類似。

Primary hyperparathyroidism was once considered as a rare disease whose diagnosis was based on the classical symptoms of “bone, stones and abdominal groans” More recently, however, the routine quantitaion lcium by automatic methods has shown this to be a more common disease in adults. Although primary hyperparathyroidism can occur in children and adolescence, current evidence confirms that it remains a rare disease in this age group. While most cases of the primary hyperparathyroidism in neonates and infants are caused by parathyroid hyperplasia, the majority of cases appearing during childhood and adolescence are caused by a parathroiid adenoma. This report of a child with parathyroid adenoma presented with bilateral femoral neck fractures with were initially tought to be pyarthroses. The roentgenogram of this ailing gril showed a very pronounced dense metaphyseal band over upper and lower ends of the tibia and fibula. Reviewing the literature, we can not find any other case with had so prominent picture as we have.
