Twenty patients who had painful valgus gonarthrosis received a distal femoral varus osteotomy between October 1998 and June 1995. Thirteen osteotomies were done for osteoarthrifis; 5, for post-traumatic arthritis; 1, for rheumatoid arthritis; and 1, for poliomyelitis with deformity. The mean age of the patients was 49 years (range, 27 to 62 years). The length of follow-up averaged 50 months(range, 24 to 102 months). The preoperative average tibiofemoral angle was valgus 18.5 degrees. The average tibiofemoral angle at recent follow-up was valgus 1.5 degrees. At recent follow-up, 17 patients (85%) had good or excellent results. Three patients had a less satisfactory result which was attributed to tricompartmental disease of the knee and suboptimal correction of the deformity. Other complications included 1 nonunion, 1 delayed union, 1 broken screw and 1 ipsilateral tibial fracture. Seven patients had a patellofemoral arthrosis including 2 dislocations of the patella and all had satisfied with the result. We concluded that distal femoral varus osteotomy is an effective surgical procedure for valgus gonarthrosis if we select the patients properly (excluding the tricompartmental disease).
