Severe Community-Acquired Pneumonia Due to "Legionella pneumophila" in a Chronic Peritoneal Dialysis Patient

"Legionella pneumophila", a relatively rare bacterial pathogen of community-acquired or nosocomial pneumonia, can cause severe illness and death if not treated properly. All reports of legionella pneumonia in uremic patients have focused on hemodialysis, but none on peritoneal dialysis, Here, we report the firstcase of severe community-acquire d Legionnaires' disease in chronic peritoneal dialysis patients of Taiwan . He was admitted to the intensive care unit and his pneumonia resolved gradually after treatment with levofloxacin, followed by erythromycin for one month. Legionnaires' disease is not readily recognizable by clinical manifestations or laboratory tests; hence, "L. pneumophila" should be included in the differential diagnosis of severe pneumonia, especially in immune-compromised host s like dialysis patients. Early diagnosis and appropriate antibiotics therapy can reduce mortality.
