Tumour development is a multi-step process during the transition from a normal to a malignant cellular state. In the past decade, extensive effort has been made not only to define the molecular mechanisms underlying progression to malignancy but also to predict the development of the disease and to identify possible molecular targets for therapy. Plasmingen activator system is believed to involve in this process. However, the mechanism of PA system is still controversial. When a normal cell starts to be dysplastic, both uPA and tPA activators seems to participate the prcess and lead to the cell undergo undifferentiation whereas the uPAR may participate in the initial events of tumor invasion. High levels of PAI-1 were seen from the low-grade tumour stage to tumor metastasis. PAI-2 was found mostly in late stage of tumour growth and invasion. Among these, many of them were also found to have prognostic values. High uPA, uPAR, PAI-1 and tPA/PAI-1 levels were associated with poor prognosis or RFS and OS whereas elevated tPA, PAI-2 and the uPA/PAI-1 complex level is correlated with a better RFS and OS. Therefore, further studies should be made to determine the usefulness of the application of these indicators.
