心臟衰竭檢測的新生物標誌:B型利鈉利尿胜肽(BNP)與心臟衰竭的相關研究

New Biomarker for Heart Failure Diagnosis: BNP and Heart Failure Correlation Study

摘要


心臟衰竭是一個死亡率高,且不容易診斷的疾病。而B型利鈉利尿胜肽(B-type natriuretic peptide, BNP)是在1988年所發現由心室所分泌的生物標誌。因此爲了瞭解B型利鈉利尿胜肽與心臟衰竭之間的關係,針對疑似心臟衰竭的200名患者來檢測血中的BNP的濃度。結果顯示,有心臟衰竭的患者,其血中BNP的濃度明顯較沒有心臟衰竭的患者高(1168.9pg/ml v.s. 49.3pg/ml)。而在性別的部分,無論有無心臟衰竭,女性的濃度較男性爲高,不過差距並不大。在年齡部分,無論在哪個年齡分組,有心臟衰竭疾病的患者,其BNP的濃度皆明顯比沒有心臟衰竭的患者爲高。最後,在BNP的敏感性、特異性、陽性預測值、陰性預測值與臨床一致性的部分,BNP的敏感性93.6%,特異性爲89.0%,陽性預測值爲91.1%,陰性預測值爲92.0%,臨床一致性爲91.5%。經由以上分析顯示,BNP對於臨床在心臟衰竭的診斷上是很有幫助的。

Objective: Heart failure (HF) is a disease with high mortality and not easy to be diagnosed. Identified in 1988, B-type natriuretic peptide (BNP) is a new biomarker secreted from the myocytes located in the left ventricle. In order to have a clearer scope in the relationship between BNP and heart failure, the level of BNP was measured in 200 heart failure suspected patients and analyzed. The result demonstrated that the blood BNP levels are higher in HF patients than in non-HF patients (1168.9 pg/ml v.s. 49.3 pg/ml), and this did not differ between age groups. Besides, the blood BNP level was slightly higher in female patients than in male patients regardless the diagnosis of having HF or not. Finally, the sensitivity of the BNP result in this study was 93.6%, specificity was 89.0%, the positive predictive value was 91.1%, negative predictive value was 92.0% and the overall clinical correlation was 91.5%. In short, BNP was shown to be of valuable clinical significance in the diagnosis of HF in this study.
