Penile Fracture: A Rare Case of Simultaneous Rupture of the Corpus Cavernosa, Corpus Spongiosum, and Penile Urethra

Penile fracture used to be regarded as a rare disease with onlyh 250 cases reported by Mydlo in 1998 [1]. However, in a recent review, Eke estimated that over 1300 cases had been reported during the last 65 years [2]. Despite the increased reports of penile fractures, fewer than than 10 cases of simultaneous rupture of the urethra and both the corpus cavermosa and spongiosum have been reported [3,4]. A 52-year-old male patient suffered tremendous pain and rapid detumesence while engaging in vigorous sexual intercourse with his wife. After history taking and a physical examination, penile fracture was suspected and was confimed by cavernosography and color Doppler ultrasonography. Bleeding was also detected on the urethral meatus; hence a retrograde urethrogram showed contrast medium extravasstion from the penile and bulbous urethra. Immediate exploration revealed a large hematoma at the penile base, simultaneous rupture of both the corpus cavermosa and spongiosum along with the complete rupture of the penile urethra. Bilateral corporrhaphy and reanastomosis of the penile urethra were performed with a smooth postoperative recovery. In our review, the pathogenesis and current treatment for penile fracture are discussed.
