摘要
腎嗜酸細胞瘤(oncocytoma)是由腎近曲小管上皮細胞贅生的良性腫瘤，有包覆纖維莢膜界限分明，其中央區有明顯的纖維化疤痕組織，腫瘤本身無壞死或出血現象。血管攝影檢査，在動脈相可見到輻射車輪狀(spoke-wheel)的血管向腫瘤中央集中，靜脈相可見到似腎實質性均勻塗染，這種特殊的血管形態，可幫助作爲與腎細胞癌之鑑別診斷。此種腫瘤多發生於中老年人，本文報告一病例年齡27歳，主要症狀爲突發無痛性血尿。從文獻之報告，此種病例極爲少見。

Renal oncocytomas are benign tumors arising from proximal renal tubular epithelial cells. They usually occur in old-aged people. They appear radiographically as solid masses which are vascular on angiography and sometimes may be confused with renal cell carcinoma. The distinctive appearance of this lesion is a well-circumscribed, uniform tan-brown tumor with a prominent central scar and lack of necrosis or hemorrhage. The characteristic angiographic findings for the oncocytoma include a spoke-wheel pattern in arterial phase and a homogeneous blush on the nephrogram. The CT scan findings in our case are well-defined tumor with relatively low attenuation value and slightly enhanced after intravenous contrast medium administration. Band-like hyperdense shadows in the central portion of the tumor are actually fibrotic scars. Less than 60 cases have been reported in the English literature and has not been reported in Taiwan. Recently we find a case of renal oncocytoma which has classical angiographic findings and can help us to differentiate it from renal cell carcinoma.
