由於高解像力超音波掃描儀之發展,對於甲狀腺病灶之解剖關係位置,以及病灶本身回音特性有極為清晰之描述,故在偵測病灶上愈顯其高靈敏度(high sensitivity),但是對低功能性單一性固質甲狀腺結節(hypofunctioning solitary solid nodular lesion)之鑑別診斷上尚有其困難,其原因乃由於良性與惡性病灶在超音波形式上之相似性,至於甲狀腺囊腫內乳突樣腺癌之表現上並無文獻報導其特殊之超音波形式,我們在最近證實了一個罕見的甲狀腺囊腫內乳突樣腺癌,其超音波形式表現為囊腫樣病灶內有固質高回音性乳突樣腫塊,不隨姿式改變而移動,並且表面有指狀突起,我們認為此等發現為甲狀腺囊腫內乳突樣腺癌極具意義之現象,故提出此病歷報告。
Owing to the recently developed ”high-resolution” ultrasound scanner, the anatomical localization and the echo-pattern of a thyroid lesion may be clearly demonstrated. Higher sensitivity may be therefore achieved in detection of thyroid lesions. However, difficulty is still encountered in the differential diagnosis of a hypofunctioning solitary solid nodualr lesion because of the similarity in echo-patterns between benign and malignant lesions. There is no well-documented sonographic appearance in papillary carcinoma arising from a thyroid cyst. We recently encountered a surgically and pathologically proved case with this situation. Her sonograms showed a solid mass with frond-like surface arising from the wall of a cystic thyroid lesion. The mass did not move during the patient changing her position. We believe these findings are of sonographic significance for papillary carcinoma arising from a thrroid cyst.
