骨盆骨折之病人在傳統骨盆X光攝影往往不能很精確的顯示全部的損傷情形,尤其在髖臼、股骨頭、髖關節腔、骶體關節(sacroiliac joint)和軟組織,常因為不能完全準確瞭解全部骨折情形,以致影響到治療方式的選擇,而電腦斷層的影像除了可以完整的顯示整個骨盆骨的解剖結構外,對於骨盆骨折的診斷則有下列好處:(1)顯示髖臼骨折的全部情形。(2)顯示髖關節腔之關節游動體(loose body)。(3)顯示骶髂關節骨折和分離。(4)顯示骨盆周圍軟組織損傷和血塊腫(hematoma)。三軍總醫院自74年7月至74年12月共有20例因意外傷害造成骨盆損傷的病人接受電腦斷層檢查,電腦斷層影像皆提供了正確的診斷,包含隱藏性骨折10例、骶髂關節分離3例、髖關節腔的關節游動體10例、骨盆腔血腫塊6例,本文將就其電腦斷層影象及傳統放射線學診斷作一分析與比較。
The condition of acetabulum, femoral head, hip joint space and sacroiliac joint is. frequently difficult to evaluate on conventional radiographys after pelvic injury, which influences the selection of treatment. The CT images offer helpful in (1) delineating detail fractures of the acetabulum. (2) showing bony fragments within the joint space. (3) showing seperation or fracture of sacroiliac joint. (4) demonstrating the extent of soft tissue injury and hematoma. The CT and plain film examination were compared in 20 consecutive adult patients post pelvic injury from July 1985 to December 1985. Retrograde analysis of all cases was done in this report.
