RADIOLOGICAL EVALUATION OF THE PRIMARY SMOOTH MUSCLE TUMORS OF THE GASTROINTESTINAL TRACT

消化道平滑肌腫瘤之放射線診斷

摘要

The radiological features of 30 cases of smooth muscle tumors of the gastrointestinal tract, at our institution, during the period of March, 1984 to March, 1988 are reviewed. It includes twenty cases of leiomyomas, one case of leiomyoblastoma and nine cases of leiomyosarcomas. Their special appearances on the barium examinations. C. T. and angiographic examinations are studied. The complications, metastasis, recurrence and differential diagnosis are discussed. All cases are proven histologically by the pathology department.

平滑肌腫瘤（smooth muscle tumor）在消化道上是一種少見的腫瘤。雖然，確立診斷需要靠病理上的證實，但由於它們在放射線學上顯示的特性，早期的診斷，有賴於放射線檢査的幫助。馬偕醫院從民國七十三年三月至七十七年三月的四年期間，經過組織病理學證實之消化道平滑肌腫瘤共有30例。包括平滑肌瘤（leiomyoma）20例，平滑肌胚細胞瘤（leiomyoblastoma）1例，平滑肌肉瘤（leiomyosarcoma）9例。本文依其在鋇劑X光檢査，電腦斷層攝影和血管造影上的特殊徵象加以分析，結果顯示鋇劑X光檢査對於黏膜下的病灶（submucosal lesion）能作有效的偵測，電腦斷層攝影對於腫瘤本身的診斷和分期甚具價值，而血管造影檢査更具有特異性和敏感性，尤其是在鋇劑X光檢査未能偵測出來的小腸平滑肌腫瘤。另外，我們在其併發症，復發，轉移和鑑別診斷上，就我們的經驗和參考文獻上的報告提出討論。
