This article depicts the radiographic characteristics of cystic renal cell carcinoma (RCC); it also correlates radiographic characteristics with surgical-pathologic findings. Five cases of RCC that manifested as complicated cyst were presented. Based on the Bosniak classification, there were four cases of Category Ⅳ lesion (enhancing mural nodularity in three cases, enhancing solid component in one case), and one case of Category Ⅲ lesion caused by thickening of intervening septum in a multilocular cyst. Histopathologic foci of RCC correlated well with the location, radiographically, of enhancing mural nodularity, solid component and thickened septum. When dealing with a cystic lesion classified as Bosniak Category Ⅳ one should be aware of the possibility of associated malignancy.
