針織物設計對於乳癌放射線治療乳房托高與穿戴穩定性及舒適性之影響

Effects of Knitted Fabric Design on Breast Positional Stability and Wearing Comfort of Personalized Breast Holder for Women Receiving Whole Breast Irradiation

摘要


台灣乳癌的發生率逐年上升,發生率排名為女性癌症第1位,並且有年輕化的趨勢。而初期乳癌病患在接受部分乳房切除術之後,為了降低復發的機率與提高存活率,大多會在術後接受放射線治療。為降低乳癌病患於放射線治療時心臟與肺臟所接收到的輻射劑量,進而減低心臟與肺臟的受損,故使用特殊輔助穿戴織物,故本研究主要探討針織物設計對於乳癌放射治療時乳房托高、穿戴穩定性及穿戴舒適性之影響。本研究首先採用雙面組織,內層皮膚接觸層材質分別選用聚酯纖維與Tencel兩種紗線,搭配不同條數熱熔紗的定型外層,以橫編織造單片試樣,並以三種不同溫度定型,先了解在不同的定型溫度下,其抗拉伸強度,作為織物支撐能力的依據。再根據90度半俯臥屈身乳房掃描所得之形狀,規劃成型試樣之引返位置與數量的版型設計;並藉由抗拉伸強度結果,規劃熱熔紗不同條數的分佈設計,將版型設計與分佈設計組合成三種不同的針織設計,並用成型方式織造,以三種不同溫度定型後,將其固定於包含肩帶、背片、膠條與鉤扣且左胸挖空的底部內衣。最後,經受測者平躺實測穿戴後,得出設計一在100 ℃定型溫度下,其對於乳房托高效果與未穿戴對比可增加3.1公分,整體穿戴穩定性於定位量測的位移皆在±0.2公分內,舒適性的部分則採主觀評估問卷,整體舒適性表現良好。

The occurrence probability of breast cancer in Taiwan is raising up yearly, especially common in women. Most of the early stage breast cancer patients would get adjuvant radiotherapy after breast-conserving surgery because it can effectively reduce local recurrence and metastasis. In order to reduce the radiation exposure and the damage of important tissues and organs, such as heart and lungs, special auxiliary wearing textile are used. In this research, we use knitting fabric as an assistive device in breast support, wearing stability and wearing comfort during the radiotherapy. Firstly, we used polyester fiber and Tencel as an inner side contacting the skin, and combined them separately with different pieces of fusible yarns which are at the outer side. After that we used three different temperatures to setting and got the tensile strength from each temperature. Secondly, according to the breast shape from a 90-degree semi-prone position, we could decide the position and quantity of flechage as the pattern design. Besides, based on the result of tensile strength, we could allocate the different pieces of fusible yarns as the distribution design. After knitting three different type of the design sample which is combined by pattern design and distribution design in three different setting temperatures, we fixed it at the undergarment which is without left cup including strap, wing, boning, hooks. In the end, after the user tried and tested while lying down, we came out that the design sample I in setting temperature 100℃ gets the best breast support. It can hold the breast by 3.1 cm compared without wearing. Wearing stability within ±0.2 cm offset according to positioning measurement in the overall design samples. As for the comfortability, we used a subjective assessment questionnaire and got a positive level of feedback for the overall comfortability.
