The regimen of choice for patients with high-risk gestational trophoblastic disease is etoposide/methotrexate/actinomycin D/cyclophosphamide/vincristine(EMA/CO). However, approximately 20-30% of patients develop resistance to EMA/CO regimen. When the EMA/CO treatment failed, it should be considered to use the combination of invasive surgery, radiotherapy and novel chemotherapeutic agents. Salvage drugs reported include cisplatin, paclitaxel, ifosfamide, carboplatin, 5-fluorouracil, bleomycin and adriamycin. There still present the substantial possibility of complete remission even in those who present with advanced stage of disease.
