一種商業上的異質填充物(alloplast)成品,多孔高密度聚乙烯(Porous high-density polyethylene (PHDPE, Medpor)),已用於矯正先天性或顏面外傷造成的顏面骨骼變形或皮下缺損。本科報告一例20歲男性,因先天性鼻樑基部(radix)過低而顯得兩眼內眥距過寬,我們以開放式鼻成形術(open rhinoplasty),選用多孔高密度聚乙烯所製成的鼻鞘(nasal shell),置入鼻背以增加鼻背高度。病患對於手街結果相當滿意,且無排斥或傷口感染現象。我們建議,使用可和人體血管纖維組織相結合、易於雕塑、且感染率低的人工合成物多孔高密度聚乙烯,是值得考慮用鈴鼻成形的村料。
Porous high-density polyethylene (PHDPE) implants, one form of commercially available alloplast, have gained wide acceptance for use in the repair of facial skeletal deformities and subcutaneous defects, including those associated with congenital anomaly or maxillofacial trauma. We were presented with a 20-year old male patient diagnosed with low radix and dorsum, which resulted in an increased intercanthal distance. We employed a nasal shell, made of PHDPE to effect nasal dorsal augmentation in open rhinoplasty and successfully corrected the patient's low radix and dorsum. The patient was satisfied with the surgical result. His postoperative condition was uneventful and no wound complications were reported. Based on our experience, we suggest that PHDPE could be considered as a good alloplastic material for rhinoplasty as it permits tissue ingrowth, is readily carved and shaped without collapsing the pores, and is associated with a low infection rate.
