Cholesterol Granuloma of Maxillary Sinus－ A Case Report

Cholesterol granuloma is a benign tumor that can be found in the head and neck region. It most commonly occurs in the middle ear, mastoid process, sella turcica, cerebellopontine angle, and apex of the temporal bone pyramid. The histopathological diagnosis describes granulation tissue composed of cholesterol clefts, foreignbody giant cells, foam cells, and macrophages filled with hemosiderin pigments. Cholesterol granuloma in the maxillary sinus is uncommonly encountered in the practice of otolaryngology. Clinical symptoms include facial pain, nasal obstruction, and rhinorrhea. However, these symptoms are not specific and are consistent with other sinonasal diseases. The recommended treatment is endoscopic sinus surgery with optional Caldwell-Luc operation. This article reports a case of cholesterol granuloma in the right maxillary sinus.

膽固醇肉芽腫為一良性的腫瘤，可發生在頭頸部位置，其中最常發生在中耳、乳突、蝶鞍、小腦橋腦角及顳骨岩部尖端等，此為病理診斷；病理組織上可見一肉芽腫內含膽固醇裂、異物反應引起的巨大細胞、空泡細胞及巨噬細胞，組織間有血鐵質色素沉澱。上頜竇膽固醇肉芽腫在耳鼻喉科臨床上並不常見，臨床症狀包含顏面疼痛、鼻塞及流鼻水，然而，這些症狀並不具有特異性且和其他鼻腔鼻竇疾病症狀類似，鼻竇內視鏡手術或合併Caldwell及Luc氏手術為建議的治療方式，我們提出了一上頜竇膽固醇肉芽腫的病例報告。
