克雷伯氏肺炎菌頸部感染－病例報告
Klebsiella Pneumoniae Neck Infection-Case Report
摘要
克雷伯氏肺炎菌（Klebsiella pneumoniae）爲人體口咽與腸道之正常菌叢，很少造成頸部之嚴重感染，然而某些抗藥性菌株因為治療困難，可能造成嚴重併發症甚至死亡。本文報告1例K. pneumoniae造成之頸部膿瘍，26歲女性，身高體重指數（Body Mass Index，BMI）34，無内科疾病或是接觸史，因右側頸部腫塊時好時壞4週而至本院急診求診，電腦斷層攝影檢查發現右側胸鎖乳突肌内側低密度塊狀結構合併環狀顯影（ring enhancement），最大徑3.8 cm。經過抗生素治療3個月後病患恢復情況良好，於門診追蹤至今無復發跡象。本文除了分享我們的治療經驗，並回顧了K pneumoniae頸部感染與頸部細菌性淋巴結感染相關的文獻。

Klebsiella pneumonia neck lymphadenitis with large abscess formation is uncommon. Increasing resistance to a broad array of antibiotics made it hard to treatment. We report a case of K. pneumoniae neck abscess in a 26 years old woman with body mass index 34 without underlying disease. She was send to our hospital due to intermittent right side neck mass for 4 weeks. Computed tomography (CT) revealed right side neck lymphadenitis with abscess formation. She recovered well after oral antibiotic therapy for 3 months. No recurrence has been observed until now. We discuss the present case and review the literature of K. pneumoniae neck infection and bacterial lymphadenitis.
