因應2019新型冠狀病毒肺炎(COVID-19)全球大流行的感染管制措施可參考疾病管制署「醫療機構因應嚴重特殊傳染性肺炎感染管制措施指引」,本文則參考美國耳鼻喉頭頸外科醫學會指引與英國耳鼻喉科學會建議,針對耳鼻喉科醫師進行內視鏡鼻竇手術(包含經鼻內視鏡顱底手術)與相關處置的特殊性提供感染控制建議。
During the COVID-19 pandemic, otolaryngologists, while performing endoscopic sinus surgery, are at particularly high risks of inflicting by the virus. Also in-office medical procedures may generate aerosol. Universal precautions are recommended to use high grade personal protective equipment (PPE), like N95 mask, goggles/face shield, gloves, and gown. Special instructions should be given to those working inside the office and operating theater. Emergent and urgent surgery need to be performed by personnel under fully equipped PPE, and after confirming the infection status of the surgical patients. Most elective and non-time sensitive procedures might be handled according to a different protocol. Considering factors like the prevalence of COVID-19, the capacity in testing patients and staff, the availability of PPE, and the workforce in surgery theater, a step wise approach is advocated in resuming elective surgeries.
