大多數耳科醫師視小耳症併先天性耳道畸型之耳道成型手術為一困難之挑戰,主因是複雜手術過程及術後高併發症。近年來,隨著各類植入型骨導助聽器之快速發展,術後聽力穩定改善及低併發症之優勢,越來越多小耳患者選擇此類產品,做為聽力重建治療方式。此外,患者聽力重建時通常同時會面臨外耳重建的考量,因此,針對個別個案之解剖條件及不同的需求,跨團隊制定個人化醫療計劃至為重要。本文將簡介現今小耳症併先天性耳道畸型的聽力重建之方式,文獻整理並提供個人經驗分享。
Most otologists regard canalplasty for microtia patients with congenital external auditory canal anomalies as a difficult challenge due to the complicated nature of the surgical procedure and the high postoperative complication rates. Recently, with the advent of implantable bone conduction implants, and the advantages of stable hearing improvement and low complication rates, more and more microtia patients accept these procedures for their treatment. In addition, microtia patients seeking hearing restoration usually have to take auricular reconstruction into consideration. It is crucial to make a personalized therapeutic strategy by a multidisciplinary team considering the individual anatomy condition and expectation. In this paper, we will briefly review contemporary hearing restoration for patients with microtia and congenital external auditory canal anomalies and present our experience.
