A case of keratoconjunctivitis sicca of both eyes with subacute onset in a 23 year-old female was reported. On Shirmer test the conjunctival sac did not wet the filter paper strip after 5 to 15 minutes at all. The conjunctival and corneal surface was dry and lusterless. The corneal surface was stippled showing tiny epithelial filaments all over it. The conjunctival elasticity was diminished. Except a mild feeling of dryness in the mouth, there was lack of associated systemic syndromes. No response was observed with the systemic treatment of predonisolone, chloroquine, Parotin, tetrahydroxy-quinone and pilocarpine injection. Frequent local instillation of 0.4% methylcellulose solution relieved the feeling of dryness and improved the visions while the eyes were wet with the solution. To achieve this the instillation had to be as frequent as once every 15 minutes. The eye findings such as staining of the conjunctival and corneal surface with rose bengal and visual acuity improved after an observation period of time for 11 year. Spontaneous improvement was likely.
