自民國65年7月至67年7月,本院發現五例因施行球後麻醉後,引起全身類似癲癎大發作的病例。此種類似癲癎大發作及呼吸停止的合併症是少見的。探討其因,最可能是麻醉劑,直接注入眼動脈或其血管鞘。然急早發現,緊急急救,預後良好。
Five patients who had suffered from grand mal like seizure and apnea after retrobulbar anesthesia were found in General Veterans Hospital from July, 1976 to June, 1978. Most part of opththalmologists can carry out the operation under retrobulbar anesthesia. 2% xylocaine is the most popular useful local anesthetics. Grand mal like seizure and apnea are very rare complications and not often seen in retrobulbar anesthesia. The probable cause is to inject xylocaine accidentally into the ophthalmic artery or its sheath. Then sudden increased concentration in blood stream causes toxic reaction with evidence of cortical stimulation and medullary depression. In treatment of apnea, artificial ventilation must be instituted immediately either through mouth to mouth methods, bag and mask or an endotracheal tube. Control of convulsion by means of an ultrashortacting barbiturate or valium intravenous injection is indicated. Prognosis is ecxellent if treatment starts earlier.
