摘要

本篇報告依爾士病之三例病患四隻眼睛之玻璃體手術之治療經過及結果。此四隻患眼皆有嚴重之玻璃體出血，其中兩隻眼睛有網膜前纖維膜，一隻眼睛有牽引性網膜剝離，經過長期觀察，視力皆無進步之跡象。經施行玻璃體手術後，追踪其視力分別進步到6/4.5，6/7.5，6/20，3/60。其中視力較差之病例是因為黃斑部網膜皺縮引起。著者等認為玻璃體手術對嚴重之依爾士病乃為一有效之症狀療法。

The clinical courses and results of four eyes from three cases of Eales' disease treated with pars plana vitrectomy are presented. All these four eyes were complicated with severe long-standing vitreous hemorrhage, with two of them associated with thick pre-retinal fibrous tissue and another one with traction retinal detachment. The surgical procedure consisted of complete removal of blood clots & cryopexy in all cases and peeling & circumcision of fibrous tissue in two. The surgical complication was an iatrogenic retinal tear occuring in one case during the dissection of fibrous scar which was successfully treated with seleral buckle. The visual acuities of these four eves postoperatively were 6/4.5, 6/7.5, 6/20, 3/60 respectively. The cause of poor vision was due to the macular pucker which was left behind after the peeling of fibrous membrane. Eales' disease almost exclusively occurs in the young productive age group, propel management is most important in the socio-economical standpoint. The authors conclude that vitrectomy surgery offers a most effective symptomatic treatment for Eales' disease.
