全身麻醉對基礎淚液分泌之影響

The Effects of General Anesthesia on Basic Tear Secretion

摘要


三軍總醫院眼科選擇三十個全身麻醉的病人,測量基礎淚液分泌試驗。以Alcon所出產的Schirmer Tear Teat Strips在病人麻醉前之清醒狀態下及放入氣管插後之十分、三十分及六十分鐘,共四次測量其基礎淚液分泌。結果顯示在穩定的麻醉狀況下,病人接受麻醉程度愈深,時間愈長,其基礎淚液分泌就愈減少,因此在全身麻醉的時間要長,麻醉劑量需深的手術,為了維護角膜正常生理狀態,應該給予病人眼睛防護性之眼藥膏,同時將上下眼瞼以膠布貼台,以保護角膜的安全。

Observations of amount of basic tear secretion have been carried out on 30 surgical patients under endotracheal general anesthesia. Standardized Schirmer tear test strips (Alcon) was employed for the measurement. The basic tear secretion was measured before the general anesthesia, then repeated at intervals of 10, 30 and 60 minutes following endotracheal intubation. It has been observed that as the effect of anesthesia went deeper and the longer was the duration of anesthesia, the amount of tear secretion became less. It is suggested that we should apply non-polar ophthalmic ointment and approximation of the lids with tapes during major and long surgery to prevent damage during general anesthesia.
