本報告係收集48個病例共50眼。所做的手術包括白內障、青光眼、視網膜剝離、眼球內異物、玻璃體纖維化等。每術前施以無併用epinephrine、hyaluronidase等比例混合之2% Lidocaine及0.75% Bupivacaine Hydrochloride做眼球後麻醉及瞼麻醉。使用劑量約8至10c.c.。結果顯示,其作用發生時間比單獨使用Bupivacaine Hydrochloride迅速,並且仍然具有長效作用。
A study was made of combined 0.75% bupivacaine hydrochloride with 2% lidocaine for anesthesia in 50 eyes of 48 cases undergoing surgery of cataract, glaucoma, retinal detachment, intra-ocular foreign body and vitreous fibrosis. In addition to the above mentioned anesthetics, no epinephrine and hyaluronidase were further combined. The total dose employed ranged from 8 to 10 ml for akinesia and retrobulbar anesthesia. The onset of anesthesia was earlier than akinesia in all cases. By 2 hours after injection of the anesthetic solution, only 6 eyes had mild sensory return. The onset of akinesia ranged in time between 5 and 20 minutes among 45 eyes, 90%. The onset time was less than 4 minutes in 2 eyes and longer than 25 minutes in 3 eyes. By 2 hours after injection mild motility return was observed only in 5 eyes. In all cases the onset of anesthesia was earlier than akinesia. It provided ocular anesthesia and akinesia of 2 to 3 hours’ duration, as plain bupivacaine did, among the great majority of cases. The combined use of bupivacaine hydrochloride and lidocaine provided more rapid onset of anesthesia and akinesia than plain bupivacaine. Its effect of anesthesia and akinesia was of longer duration than plain lidocaine.
