視乳頭環線血管裸露及早期視野變化
Baring of Circumlinear Vessel and Early Visual Field Change
摘要
視神經乳頭凹陷之擴大，在早期之診斷極為不易。若能找到凹陷綠有環線血管存在，而此環線血管又是裸露，在診斷上極有幫助。 本篇統計門診患者500眼及青光眼檔案420眼，只有50%患眼有一條以上之環線血管存在，其中只有40眼有裸露之環線血管出現，比例甚低。 早期視乳頭凹陷擴大不一定有視野缺損。40眼中只有4眼可見鼻側階梯狀之視野缺損。反之已有凹陷擴大併視野缺損之患眼，雖有環線血管存在，可能呈扭曲狀(bayoneting)而不裸露。 環線血管裸露之診斷相當不容易，但若見此現象，可視為早期之預警，小心追踪觀察病人是必須的。

Early diagnosis of a pathologic enlargement of the optic disc cupping is difficult. If we can find the baring of the circumlinear vessel which was described by Herschler & Osher in 1980, it may be helpful for the recognition of the enlargement of the cup. We studied 500 eyes in OPD and 420 eyes on glaucoma filed color slide. The presence of the circumlinear vessel was only 50% of the total eyes. 40 eyes (8.6%) of the 50% were noted baring of the circumlinear vessel. Early enlargement of the disc cup may be present before visual field loss. In our study we found 4 eyes with nasal step like field defect which corresponded to the baring of the circumlinear vessal. Although we agree that to recognize the baring of the circumlinear vessal is difficult, it is still worthful for noticing the early change of the optic disc cup.
