就榮民總醫院及台中分院眼科25年間經病理證實之眼窩腫瘤200例(男106例,女94例;年齡2至72歲)作一統計分析。臨床主要症狀為眼球突出、視力減退、可觸摸腫塊、眼球移位、複視、眼瞼腫脹、眼球或眼球周圍疼痛、眼底變化。眼窩原發性腫瘤佔61%,鄰近組織侵犯性腫瘤佔28.5%,全身性或轉移性者佔10.5%。良性腫瘤佔67%,惡性者33%。最多見之眼窩腫瘤依次為血管瘤(11%)、淚腺瘤(10.5%)、皮樣囊腫(9.5%)、粘液囊腫(7.0%)、偽腫瘤(6.5%)、鼻竇癌(5.5%)、淋巴瘤或白血病(5%)、類淋巴增生(4.5%)、腦膜瘤(4.5%)、神經鞘瘤(3.5%)、網膜母細胞瘤(3.5%)、鼻咽癌(3%)。原發性瘤以血管瘤最多見;續發性瘤以粘液囊腫侵犯最多見;轉移性癌以神經母細胞癌最多(4例),乳癌、肺癌、肝癌各2例。治療以手術摘除為主(73%),放射治療次之。治療結果:治癒者53%,復發者13%,死亡者8%,不詳者26%。淚腺瘤病例(2l例)及鼻咽癌病例(6例)較多,似乎是東方人尤其中國人眼窩瘤的特點。視網膜母細胞癌(7例,佔3.5%)佔眼窩瘤比率雖不高,卻也值得警惕。
A series of histopathologically proved orbital tumors from 1960 to 1985 at the Veterans General Hospital of Taipei and Taichung, Taiwan, including 200 patients (aged ranging from 2 to 72 years old) is reviewed. Of the 200 cases, tumors were more common among males than females (106:94). The main symptoms of orbital tumors were proptosis (82%). blurred vision (58%), palpable mass(39%), double vision (33%), eyelid edema (31%), ocular or periocular pain (24%). 61% of the tumors were primary in origin, 29% were invasive, and 10% were metastatic or systemic. 67% of the tumors were benign, and 33% malignant. The most common primary orbital tumor was hemangioma. The most common secondary (invasive) orbital tumor was paranasal sinus mucocele. The metastatic orbital tumors included neuroblastoma (4 cases), breast cancer (2 cases), lung cancer (2 cases), hepatoma (2 cases), renal cell carcinoma (1 case). The first 10 most common orbital tumors were: hemangioma (11%), lacrimal gland tumor (10.5%), dermoid cyst (9.5%), mucocele (7%), pseudotumor (6.5%), carcinoma of paranasal sinus (5.5%), lymphoma or leukemia (5%), lymphoid hyperplasia (4.5%), meningioma (4.5%), neurilemmoma (3.5%). In adults, the first 5 most common orbital tumors were: lacrimal gland tumor, hemangioma. mucocele, pseudotumor, carcinoma of paranasal sinus. In childhood (younger than 14 years old), the first 5 most common orbital tumors were: dermoid cyst, retinoblastoma, leukemia, hemangioma, neuroblastoma. Seventy three percents of 200 patients had their tumors surgically removed, 26% received radiotherapy and 7% received chemotherapy. Of the 148 cases followed adequately, 107 were cured, 16 died and 25 recurred. In literature on the subject, lacrimal gland tumors are suspected of being more common in Orientals than the Occidentals, and this study's results would also seem to support these suspicions (10.5%, ranked 2nd in frequency). The greater prevalence of nasopharyngeal carcinoma (6 cases) in this series warrants further investigations.
