Flash VEP對白內障病人視力預測之評估

The Predictive Value of the Visual Evoked Potential in Patients with Cataracts

摘要


本報告對53人70眼老年性白內障患者術前做閃光視覺誘發電位(flash VEP),根據所得誘發電位之潛值(latency)、波幅(amplitude)、波形(waveform)與42眼視力1.0以上之正視眼所得誘發電位之正常值對照復加以分組,並與患者術後至少追踪二個月以上的矯正視力做比較,尋求flash VEP與白內障術後視力之開係。結果顯示P100 component之潛值和波幅都具有預測價值,尤其是二者合併判讀時。

Flash visual evoked potential from the occiput was recorded from 70 eyes of 53 patients with senile cataracts before surgery and was compared with the postoperative visual function in order to evaluate the potential vision of cataractous eyes and also to predict the postoperative vision. VEP was recorded by monopolar derivation with an active electrode placed on the site 2 cm above inion. with the reference electrode placed on the vertex and the earth electrode placed on the middle of forehead. Both latency and amplitude of P100 component from flash VEP were classified into normal (Ⅰ), subnormal (Ⅱ), abnormal (Ⅲ) groups based on the criteria of whether the photopic component (P100) was within the mean ± 2sd of normal value and whether the waveform was detectable. Combined simutaneous readings of latency and amplitude disclosed a postoperative vision of 0.3 or better in the normal and subnormal groups but of less than 0.3 in the abnormal group. Flash VEP, unlike pattern VEP, can only reflect average readings of the macula cone system to the visual cortex. But its convenience and usefulness in the evaluation of the potential vision of cataract patients make it recommendable.
