1972年,Ellenberger將opsoclonus、ocular dysmetria及flutter-like oscillations of the eyes三種少見的特殊眼球運動,總稱為眼球運動失調症(ocular dyskinesia)。本文報告一例特殊異常眼球運動之病例,利用電動眼震圖(electronystagmography)記錄該病人眼球運動之詳細變化,發現該病人之眼球運動屬於to-and-fro,back-to-back saccades without an intersaccadic interval之變化,屬flutter-like oscillations of the eyes。
Being recognized signs of a limited number of diseases involving the cerebellum and possibly the brain-stem, opsoclonus, ocular dysmetria, and flutter-like oscillations of the eyes were termed as ocular dyskinesia by Ellenberger in 1972. Ocular dysmetria is manifest in the eyes by an overshoot on attemped fixation and consequent irregular oscillations of the eyes on changes of fixation. Flutter-like oscillations refer to intermittent to-and-fro oscillations of the eyes lasting not more than a few seconds, often associated with changes of fixation but frequently appearing to be spontaneous. Opsoclonus means irregular and continual oscillations of the eyes, which differs from the flutter pattern in being more sustained, totally irregular, and occurring in the vertical as well as the horizontal plane. A 58-year-old woman visited our clinic with the chief complaint of headache, dizziness and abnormal eye movement. Physical and neurological examinations and CT scan of the brain were normal except oscillations of the eyes. Flutter-like oscillations of the eyes was diagnosed via the help of electronystagmogram (ENG) in which the flutter pattern presented throughout the procedure.
