本篇報告3名正常捐贈眼球及9名慢性閉鎖性青光眼的小梁切除術標本之傳送電子顯微鏡構造。慢性閉鎖性青光眼之小梁明顯增厚,融合及纖維化,小梁間的空間減少,小梁中膠原及似彈性纖維增加,捲曲膠原也增加,小梁內皮細胞減少,舌唾細胞及色素量未見特異情形,Schlemm's canal的管徑大小,內皮細胞及大空泡數量較少或正常,變異性大,Schlemm's canal內皮細胞之基底膜較厚,cribriform plate較纖維化,角膜鞏膜小梁與葡萄膜小梁變化類似,但後者的小梁排列較疏鬆,且色素較多。
The ultrastructure of trabeculectmy speccimens obtained from 9 chronic angle closure glaucomatous eyes was compared to that of 3 donor eyes. We studied the trabecular meshwork from Schwalbe's line to the iris root, the canal of Schlemm, its inner wall and outer wall with transmission electron microscope, our findings of the structural alterations included thickening and fusion of trabeculae, cell loss from trabecular meshwork, abundant extracellular matrix in the endothelial meshwork. There was no obvious difference between the normal and glaucomatous eyes in the features of the pigmentation, the number of canalicular endothelial cell, the giant vacuoles and the caliber of the Schlemm's canal. It was suggested that the structural alterations of trabecular meshwork produced an occlusion of the pathways for aqueous passage and resulted in glaucoma.
