眼軸長度對各類人工水晶體度數計算公式之差異分析

摘要


本文選取473個白內障摘除手術合併人工水晶體植入,並追蹤二個月以上者,測量其術前角膜曲度,眼軸長度及前房深度,計算出11個人工水晶體度數計算公式之正視眼所需人工水晶體度數及手術後屈光度數估計值,再與手術後屈光度數值做比較,藉此評估分析11個公式對於長眼軸,短眼軸及中等眼軸病例之人工水晶體度數計算的準確性。我們發現第一代理論光學公式在短眼軸群有高估人工水晶體度數,而在長眼軸有低估度數的現象。在改變眼軸長度或角膜曲度時,依據理論光學參式計算出的結果影響較大,依據統計迴歸公式計算出的結果,影響較小,大多數的第二代人工水晶體度數計算公式都可避免較大的術後屈光度誤差。

The intraocular lens power of 473 pseudophakic patients were re-calculated by using 11 different formulas to evaluate the accuracy and precision of each formula. Theoretical formulas showed over-estimation in short visual axis (<22mm) but under-estimation in long visual axis (>24.5mm). Regression formulas were less relative to the change of the biometric data. Most of second generation formulas have significantly decreased the postoperative refractive error.
