Reconstruction with Proplast in Isolated Medial Orbital Wall Fracture with Enophthalmos
孤立性之內眼窩壁骨折併眼球內陷以Proplast重建
摘要
本篇報告二例臨床上少見之孤立性眼窩內壁骨折，其臨床表現：第一例為輕微之水平複視，合併左額眉部之下垂，明顯的左眼內陷；第二例為明顯之右眼內陷。除此之外，二例均無明顯之眼球運動障礙，亦無垂直複視之主訴。經由電腦斷層掃瞄，骨折之部位，軟組織嵌塞之程度皆獲得精確之診斷。 鑑於以往重建術所使用切痕之方式，眾說紛紜。本篇強調由於Proplast本身優異之生物相容性，體積、形狀之可鍛折性，故主張由內眥切痕並以Proplast重建，以期最小之組織損傷。術後可見，二例臨床症狀均獲得改善，植入之Proplast無脫出或感染之跡象。

Two cases of unusual presentation of an isolated medial orbital wall fracture are described. Both of them show enophthalmos with no significant restriction of extraocular movement. CT-scans demonstrated a comminuted medial orbital wall fracture, with the medial rectus muscles herniated into the fractured sites, but not incarcerated. They are reconstructed with proplast using a medial canthal incision. Enophthalmos was improved after the reconstructive surgery.
