Cyclic Oculomotor Paralysis: Case Report
摘要
週期性動眼神經麻痺(Cyclic oculomotor paralysis)相當罕見，其病因不明，是一種良性的運動性疾病，常被誤診為先天性動眼神經麻痺。本病例報告一位二十一歲男性，自出生起左眼即呈現典型動眼神經麻痺期(paralytic phase)及痙攣期(spastic phase)規律地交替運行。

A 21-year-old male with typical oculomotor paralysis of left eye is presented and video-recorded. The etiology and pathophysiology of the syndrome are unknown. The cyclic changes that occurred were pathognomic. Durng the paralytic phase, the lid was ptotic, the pupil was dilated (7mm) and unresponsive to light, and the globe was displaced down and out (70-80 prism diopters). In the spastic phase, the pupil constricted (1.5mm), the eye opened and the globe returned to midline. The total pupil cycling time was about 60 seconds and the duration of each phase was approximal equal. At the beginning and at the end of the spastic phases, the left lid showed a number of twitch-like jerks. Recession of LRM and resection of MRM were performed with an acceptable result but the cyclic lid retractions were still noticeable.
